If you want to have your own custom logo used on the site, please upload it here. Otherwise we will use the default Cook For Life logo.
If you would like to use a custom color to match your logo that will replace the blue used throughout the site, please enter the color code here. If you don't know the color code, please describe the color.

Once you submit the completed version of this form your personalized website will be up and running in less than 5 business days.
